Chronic Care Model

All of MaineHealth's clinical integration programs are based on the chronic care model developed by Improving Chronic Illness Care, a national program of the Robert Wood Johnson Foundation and Group Health Cooperative. The model summarizes the basic elements for improving care in health systems and the community, organization, practice and patient levels. The model draws on the best from research and practice, and provides a foundation for collaborative programs, tools, and research aimed at improving care for the chronically ill.
