Introduces the Patient Centered Medical Home concept, describe the benefits of practice recognition and improvement, and describe the tools and documentation used in the application process. It will also address the following questions: What is a Patient Centered Medical Home? What are the benefits for me and my practice? What is NCQA PCMH Recognition? How can my practice apply for NCQA recognition?
